Dr. Werely does not participate with commercial insurance plans, Medicare or Medicaid. An itemized statement and receipt of payment will be provided at time of encounter that you may submit to your insurance plan for reimbursement per out-of-network benefits.

Review testing, discuss new symptoms, questions and concerns

Botox Injection/Nerve Blocks

Botox injection fee for migraine headache (this does not include cost of product, which is covered by insurance pending medical benefits)

Independent Medical Record Review/Legal

Cost varies depending on time required to review records - Provide analysis of medical records and review diagnosis, treatment and recommended further plan.
